Request for Meningitis Record

Current and former students may request a courtesy copy of their meningitis record by completing the Meningitis Documentation Request Form. All requests will be processed on a first come, first served basis.  You will receive your meningitis record within 3-5 business days and it will be emailed to the email address you provided; we can not send your record to the university requesting it nor will a hard copy be provided to you.

If you lived on campus prior to Spring 2010 our office does not have your documentation on file as it was not required from students living on campus. If you lived on campus beginning in Fall 2009, you were not required to submit documentation for the spring. Please remember that if you have never lived on campus we will not have your meningits record.

Effective September 1, 2014, we will no longer be able to provide courtesy copies of documents submitted for spring 2010 because of records destruction in accordance with our records retention policy.
